If you or your covered dependents have incurred medical expenses while insured, please follow these steps when filing a claim:
1. Download claim form.
2. If you have other insurance, file your claim with the other carrier first. Keep copies of everything you send them, and copies of any Explanation of Benefits forms they send back to you.
3. Next, fill out your claim form. Remember — one claim form must be completed for each illness or accident. Answer all the questions on the claim form, giving complete information regarding your sickness or accident.
MAKE SURE YOU SIGN THE CLAIM FORM!
4. Attach all itemized bills, along with copies of any paperwork received from other insurance companies. We cannot process off statements; we must have an itemized bill showing a diagnosis.
